You want to know that your children will receive prompt medical attention when you're apart. Fill out this free medical release form and put your mind at ease.

For your protection:

  1. Print both pages of the medical release form.
  2. Have the consent form notarized by a notary public. Many banks offer complimentary notary services. It's an additional step, but having the form notarized ensures the medical establishment treating your child that it is, indeed, your signature on the form.
