In an Emergency, Would Your Kids Receive the Medical Care They Need? Print This Free Medical Release Form Today!

Let's face it. Emergencies happen. And as reachable as you may intend to be - through your cell phone or e-mail - it's always possible that when an accident happens, your child’s caregivers won't be able to reach you. Would they know what to do? Print out both pages this Free Medical Release Form and have it notarized by a legal notary public. Most banks will provide this service for free. It's an extra step, sure, but it provides proof to the hospital or emergency med center that it's really your signature on the form.
